Overview
Registered Nurse – Home Health is a travel assignment in Rutland, Vermont. The start date is 04/27/2026 for a 13-week assignment. Compensation ranges from $2,500 to $2,628 per week with guaranteed 37 hours per week. Shift schedule is 5x8 days. Setting: home healthcare with hospice services.

Responsibilities
- Assess patient and caregiver needs using the nursing process; develop and communicate a realistic plan of care with patient/caregiver involvement, and participate in on-call duties as required.
- Serve as Case Manager for assigned patients, actively managing caseloads and visit times in accordance with agency productivity standards.
- Complete OASIS documentation accurately and in a timely manner, staying current with changes and agency/state/federal requirements.
- Implement the medical plan of care and nursing interventions to ensure safe, effective, high-quality care; maintain infection control standards and utilize high-tech skills as needed (IV therapy, wound care).
- Collaborate with LPNs and other disciplines, coordinating patient education, and documenting needs for physical therapy, social work, and other services; communicate findings promptly.
- Ensure thorough and timely documentation across agency, state, and federal forms; support safe discharges/transfers and participate in interdisciplinary team (IDT) meetings.

Requirements
- Current registered nurse (RN) license to practice in Vermont (or eligibility per Vermont State Board of Nursing requirements).
- OASIS experience is required.
- Ability to perform high-tech nursing skills (IV use/maintenance, wound care) and uphold infection control standards.
- Ability to work in a home health/hospice setting and manage a caseload with on-call duties as required.
- Strong documentation, communication, and interdisciplinary collaboration skills.
